This form should be used for ordering a book that needs to be posted. If your are local we suggest that you obtain your book from either Monbulk Living & Learning Centre (21 Main Road, Monbulk) or Community Bank Monbulk & District Bendigo Bank (70 Main Road, Monbulk).

Postage is $17.95 for the first book + $4.00 per additional book.

Please pay the appropriate payment for book(s) and appropriate postage by direct debit to Bendigo Bank BSB: 033262 A/c No. 151312170 and include your last name as the reference once you successfully submit this form.

Remember - if you don't tell us who the payment is from in the reference using your name we will have your money but you won't have your book!!

Once the payment has been confirmed, we will complete processing of your order.
